Title III Parent Information Checklist

Identification

Give a reason for identifying child as limited English Proficient.           
Give a reason for placing child in a language instruction educational program for LEP students.           

Proficiency Level

Identify child's level of English proficiency.           
Explain how the level was assessed.           
Provide status of child's academic achievement.           

Instruction

Describe the method of instruction to be used.           
Provide a description of alternative methods.           

Meeting Needs

Explain how the program meets the child's educational strengths and needs.           

Meet Standards

Explain how the program helps the child learn English and meet academic achievement standards

           

Exiting Program

Explain the exit requirements.           
Explain expected rate of transition.           
Explain expected rate of graduation for secondary school.           

IEP

For a child with a disability, explain how the program meets the objectives of the IEP.           

Parent Rights

Provide written rights statement.           
Inform parents of their right to have child immediately removed from the program upon their request.           
Describe the options parents have to decline enrollment or choose another program or method of instruction.           
If more than one instructional program or method is offered, assist parents with selecting an option.           
Time FrameProvide information not later than 30 days after beginning of school year for a child identified for participation in language instruction program.           
For a child entering school after beginning of school year, provide information within two weeks of child's placement in program.           
Receipt of InformationProvide information in an understandable and uniform format.           
To the extent practicable, provide information in a language that the parent can understand.
